The clinic will be closing permanently on April 30, 2025. To ensure a smooth transition, please do the following before this closure.
1. Establish care with a new healthcare provider.
- After April 30, 2025, you will need to obtain medication refills and supplement management/advice from your new provider.
- If you want your records sent to your new provider, please fill out the Medical Records release form in the "Forms & Questionnaires" tab.
2. Download your health information.
- Please download any records you want to keep before April 30, 2025.
- You can download your Wellness Plans, handouts, and test results in the “Labs & Documents” tab
- You can download any invoices through the "Invoices" tab.
3. Order your supplements.
- Your Fullscript account will be active until the end of June 2025. After this time, you may obtain your supplements from your new provider, or you can find high-quality supplements through vendors such as: Alive and Well, WholeFoods Market, Pure encapsulations (pureforyou.com), or Designs for Health (designsforhealth.com).
- Please review all your supplements with your new healthcare provider.
4. Finalize our care together.
- My last patient appointment will be Thursday, April 17, 2025. If you would like to review your supplements and test reports or ask final questions, please schedule an appointment as soon as possible.
- While the patient portal will still be accessible until April 30th, the portal will only be used to clarify existing plans and will not be the forum to review detailed medical issues or test results.
